This website is an important notice from the United States District Court for the Southern District of Florida (the “Court”) regarding a proposed class action settlement and upcoming hearing in the above-referenced class action lawsuit that may affect your rights. YOUR LEGAL RIGHTS AND OPTIONS
File a Claim No later than Thursday, February 28, 2019, as defined in Paragraph 2(n) of the Settlement Agreement: If you are entitled and wish to make a claim for settlement relief, you must complete the Class Action Claim Form (“Claim Form”) and submit it to Claims Administrator or complete an online version of the Claim Form. The deadline to submit a completed Claim Form is Thursday, February 28, 2019, as defined in Paragraph 2(n) of the Settlement Agreement. (“Claim Deadline”).
Exclude Yourself by August 31, 2018: If you do not wish to participate in this Settlement, and want to be excluded from the Settlement Class, you must send a letter to the Claims Administrator, postmarked no later than AUGUST 31, 2018. Object by August 31, 2018: If you wish to object to the Settlement, you must file a written objection with the Clerk of Court and mail your objection (with the requisite postmark) to Class Counsel and Defense Counsel no later than AUGUST 31, 2018. Do Nothing: If you do nothing, you will receive no payment and will still be bound by the Court’s decisions.
